The database is so named &#8220;Respiratory Diseases Registry&#8221;. Members are delegated according to their special interest and responsible for the data collection. Report forms are available on the HONG KONG LUNG FOUNDATION Homepage (<a href=\"https:\/\/www.hklf.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">www.hklf.org<\/a>).<\/p>\n\n<p>Please visit the page and download the respective forms (in portable document format [.pdf], while the Acrobat reader can be obtained without charge) for reporting whenever you encounter the cases.
